Living Will: Unlike other advanced directives that only capture your end-of-life medical wishes, Thin Veil's Living Will is a more holistic document that includes sections on how you want to be treated throughout the end-of-life process and your desires for final disposition. This extensive form was born from death midwife, Michelle Mondia’s decade-long experience working directly with individuals going through the end-of-life process and their family members.

This Living Will is an easy-to-fill interactive digital file. Once filled, the file can be printed to be signed by you and your witness.

If at any time you would like to make a change to your Living Will you need only reopen the file, make your changes, print, and sign again. There are no limits to the number of revisions you can make and we suggest yo update your file every 3 years or as needed.

Consultation: You will be able to schedule a 30 minute consultation with Thin Veil death midwife Michelle Mondia. During this time Michelle can guide you through filling out your end-of-life documents or answer any questions that you might have about the end-of-life process.
